Health Office
The health office is available during the school day to provide services for minor injuries, illness, health screenings, and medication administration. The health office is not staffed by a Registered Nurse (RN). Services are provided by designated school staff in compliance with state law allowing non-nursing personnel to assist with medication and routine care. Your child’s health and safety are our highest priorities.
To help us care for your child, please ensure all phone numbers, email addresses, and emergency contacts are kept up-to-date in FACTS-SIS. For safety reasons, we can only release students to individuals explicitly listed on emergency and pick-up contacts.
Attendance and Absences/Tardies
The health office is responsible for tracking student attendance. If your child will be absent, please notify us each day of their absence by 8:15 AM. You can reach us by emailing attendance@stmarybashacatholic.org or calling the health office at (480) 963-4951. When reaching out, please provide your name, your student's name, their homeroom, and the specific reason for the absence. Please remember that notifying your child's teacher is not enough; you must also contact the health office directly.
If your student arrives late due to a medical, dental, or orthodontic appointment, they must bring a note from the healthcare provider for that day to be marked as an excused tardy. Please note that we cannot accept appointment reminders as valid proof. If you forget the note, you can email a copy from the provider’s office within two days to attendance@stmarybashacatholic.org Please ensure emailed notes include the student's name, homeroom, date of the appointment, and the name of the healthcare provider. Medication Policies
To ensure the safety of all students, any prescription or over-the-counter medication required during the school day must be brought directly to the health office by a parent or guardian. All medications must arrive in their original packaging with the prescribing label intact.
In accordance with Diocese of Phoenix policy, the school does not stock over-the-counter medications. Before we can administer any care, a completed Medication form must be signed by both a parent and a healthcare provider and filed with the health office. You can find the updated form below.
Hearing and Vision Screenings
Throughout the school year, our staff conducts routine hearing and vision screenings for specific grade levels following the Arizona Department of Health Services guidelines. We will contact you directly if the results show any areas of concern. If you have questions, please feel free to contact the health office.
Immunization Requirements
Arizona state law requires up-to-date immunization records or a valid exemption form with an online class certificate for all attending students. We encourage you to check your child's vaccination schedule with their doctor, especially if they are entering Preschool, Kindergarten, or turning 11 years old, as these milestones require new immunizations.
